Admissions Registered Nurse (RN) – Home Health Start of Care Specialist

Requirements
- Active Minnesota RN license in good standing.
- Ability to work independently in a fast-paced, field-based role.
- Strong time management and organizational skills.
- Reliable transportation.
Nice To Haves
- Experience with home health admissions and OASIS strongly preferred.
Responsibilities
- Complete Start of Care (SOC) admissions efficiently and accurately, balancing quality with productivity.
- Perform clinical assessments and OASIS documentation with speed and precision.
- Initiate care plans and quickly identify needed services and next steps.
- Communicate effectively with the care team to keep patient transitions moving smoothly.
- Manage your schedule and territory to optimize time, travel, and visit volume.
