About The Position

Mass General Brigham Home Care is seeking a per diem Registered Nurse to join their North Star of Care Team. This role offers the autonomy, variety, and fulfillment of home health care, allowing nurses to bring advanced clinical expertise, compassion, and innovation directly into patients’ homes. The position focuses on helping patients recover, regain independence, and manage complex conditions in a comfortable environment. The role emphasizes practicing at the top of one's license, building meaningful patient relationships, and making a significant impact. Home care nursing involves leading and coordinating individualized patient care plans, delivering 1:1 skilled nursing care, collaborating with interdisciplinary clinicians, providing advanced clinical treatments and patient education, and helping to prevent hospital readmissions and improve long-term outcomes. Nurses will guide patient recovery, empower families, and make a difference in daily lives. The Registered Nurses oversee and coordinate the full plan of care while collaborating closely with LPNs, therapists, and social workers to ensure comprehensive, high-quality care at home. Responsibilities typically include starts of care, resumptions of care, evaluations, or revisits. Patient care is delivered within the organizational philosophy, policy, and standards of community health practice. The admissions process and case management responsibilities are performed in accordance with MGB Healthcare at Home policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Skilled nursing care includes assessment, teaching, treatment, documentation, and care coordination, demonstrating high-quality customer service and financial awareness.

Requirements

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in acute care setting
  • Travel required within MGB Healthcare at Home geographic area
  • Ability to work with various computer software required
  • Strong interpersonal and customer service skills
  • Ability to work with a diverse population
  • Ability to communicate effectively in writing, verbally and electronically
  • Ability to work as a member of an interdisciplinary team of health care providers
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • Ability to organize and prioritize work, and adapt to changing situations

Nice To Haves

  • Prior home care experience preferred
  • Knowledge of hand-held patient care computer devices preferred
