About The Position

Eagleville Hospital, an independent substance use and behavioral health treatment and educational organization serving the community for more than a century, provides innovative compassionate care to those seeking treatment for stigmatized illnesses including substance use and mental health. The Admission Referral Nurse is responsible for fostering relationships, facilitate clinical review of potential admissions, coordinate admission and transfer, collaborate with all disciplines and assist with bed coordination.

Requirements

  • Graduate of an accredited school of nursing
  • Current PA State RN Licensure in good standing
  • Current Basic Life Support (BLS) Certification required
  • Minimum of 3-5 years working in behavioral health/acute care setting

Nice To Haves

  • Bachelor’s degree in nursing preferred
  • SCM Certification within three (3) months of hire

Responsibilities

  • Knowledgeable (subject matter expert) about behavioral health assessment, physical nursing assessment, nursing care, diagnosis, and interventions
  • Knowledgeable about the therapeutic milieu and interdisciplinary team approach to care and treatment (psychiatry, medicine, nursing, social work, counseling, activities, and pharmacology)
  • Discusses cases with the admitting provider and interdisciplinary team to understand their clinical impression, treatment plan and expected treatment period
  • Comprehensive Hand-off Communication/Report to receiving unit RN using SBAR tool
  • Alerts the interdisciplinary team, including the provider, of any changes in the patient’s condition upon arrival
  • Utilizes de-escalating techniques to calm patients and prevent escalation of patient’s disruptive behavior
  • Communicates professionally, clearly and completely with the patient, family, peers and interdisciplinary team.
  • Assists with the completion of admission assessments for direct admissions to the patient care units as needed or as assigned by supervisor.
  • Serve as a community liaison and maintain partnerships with referral sources within the community.
  • Participates in performance improvement (PI) activities to improve admission services
  • Complies with current Eagleville Hospital safety and infection control policies and procedures.
  • Attends and completes all mandatory education and training.
  • Other duties as assigned
