About The Position

BAYADA Home Health Care is seeking an experienced Admissions Physical Therapist (PT) for a full-time opportunity with our UCV office, providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, NJ. As an Admissions Physical Therapist, you'll play an important role in welcoming new clients to BAYADA and establishing the foundation for a successful home health experience. You'll complete comprehensive physical therapy evaluations, assess each client's functional and clinical needs, and develop individualized plans of care that help clients safely recover and maintain their independence at home. This is a great opportunity for an experienced home health Physical Therapist who enjoys clinical assessment, autonomy, care coordination, and helping clients achieve meaningful functional goals in their own environment.

Requirements

Current and unrestricted New Jersey Physical Therapist (PT) license.
Graduate of an accredited Physical Therapy program.
Minimum of one year of recent experience as a Physical Therapist.
Prior home health experience required.
OASIS experience required.
Strong clinical assessment, evaluation, and care coordination skills.
Excellent communication, interpersonal, organizational, and time-management skills.
Ability to work independently and effectively manage a field-based schedule.
Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Nice To Haves

Experience with electronic medical records (EMR) preferred.

Responsibilities

Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
Assess the client's home environment and identify potential safety concerns, fall risks, and barriers to independence.
Develop individualized plans of care based on client needs, goals, functional abilities, and physician orders.
Establish appropriate physical therapy goals and make recommendations for ongoing therapy services.
Identify additional clinical needs and coordinate appropriate nursing, therapy, social work, and other home health services.
Educate clients, families, and caregivers on mobility, safety, fall prevention, therapeutic exercises, and the home health plan of care.
Collaborate with physicians and members of the interdisciplinary care team to coordinate comprehensive client care.
Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
Accurately document evaluations, clinical findings, education, and care coordination using BAYADA's electronic medical record (EMR) system.
Help ensure a smooth transition from referral to admission and ongoing home health services.
