Admissions Nurse

St. Croix HospiceLa Crosse, WI

About The Position

The Admissions Nurse plays a vital role in hospice care as the first point of contact for patients and families facing a terminal diagnosis. In this position, you will travel to patients wherever they reside, conduct comprehensive assessments, and gather essential clinical information to ensure timely and appropriate care. You will also provide compassionate education and guidance to families as they navigate the beginning of the hospice journey. Admissions Nurses develop individualized care plans, identify immediate and ongoing needs, and help support families with clarity, empathy, and professionalism. The trust and comfort you establish during these early moments lay the foundation for a meaningful care experience, positively shaping how families move through the process with dignity, understanding, and peace. If you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we’d love to meet you.
