About The Position

The Admissions Nurse (RN or LPN) is responsible for conducting nursing care functions with a primary focus on admissions and care transitions in a long-term care/assisted living community. This role ensures safe and timely admissions, coordinates care transitions, and maintains accurate documentation. The Admissions Nurse also plays a key role in communication with referral sources, families, and internal teams, as well as medication administration and ensuring a safe environment for new residents.

Requirements

  • RN or LPN license

Responsibilities

  • Conduct nursing functions in accordance with current rules, regulations, and guidelines, with a primary focus on admissions and care transitions.
  • Ensure nursing personnel support safe, timely admissions and onboarding of new residents.
  • Coordinate and complete admissions, transfers, and discharges, ensuring accurate and compliant documentation of care and services provided.
  • Review incoming referrals and assess resident appropriateness for admission based on clinical needs, services available, and regulatory requirements.
  • Lead and participate in the admission process, including room readiness, care setup, and initial clinical coordination.
  • Participate in the orientation of new residents/family members to ensure a smooth transition into the community.
  • Collaborate with healthcare providers and interdisciplinary team members to establish initial care needs upon admission.
  • Utilize nursing personnel based on resident acuity and admission needs.
  • Review and respond to incoming referrals from hospitals and community partners in a timely manner.
  • Communicate with referral sources, families, and internal teams to coordinate admissions and care transitions.
  • Complete pre-admission assessments and initial nursing evaluations to support safe placement decisions.
  • Develop or assist in creating interim care plans/service plans upon admission.
  • Ensure all admission documentation is accurate, complete, and compliant with regulatory standards.
  • Collaborate with DON, therapy, and admissions teams to optimize census and clinical appropriateness of admissions.
  • Provide clear and timely communication regarding admission status, resident needs, and bed availability.
  • Ensure new admissions are communicated to nursing staff and that care needs are clearly outlined prior to arrival.
  • Greet and support newly admitted residents and families, ensuring a positive first impression and experience.
  • Notify healthcare providers and appropriate parties of admission status and any significant changes.
  • Continue to support interdisciplinary team communication regarding newly admitted residents and care progression.
  • Prepare and administer medications as ordered, with attention to new admissions and medication reconciliation.
  • Complete medication reconciliation upon admission to ensure accuracy and continuity of care.
  • Ensure medications, supplies, and equipment are ready and available for incoming residents.
  • Review MARs for accuracy following admissions and transitions.
  • Support nursing staff in executing smooth admissions and transitions of care.
  • Provide guidance related to admission workflows, readiness, and compliance.
  • Collaborate with DON and leadership to improve admissions processes and team coordination.
  • Ensure rooms and clinical areas are prepared, clean, and fully equipped prior to new admissions.
  • Maintain safe and compliant environments for incoming residents.
  • Identify and communicate supply/equipment needs to support admissions volume.
