About The Position

Stella Maris provides compassionate, skilled home hospice care for adults in Baltimore City, Baltimore County, and Harford County. The Admissions Nurse provides information about Hospice Home Care programs to patients, families, physicians, discharge planners, and community organizations, and admits patients to the appropriate programs. This is a full-time position working Monday through Friday with the Home Hospice team.

Requirements

• Current RN licensure in the State of Maryland.
• Prior experience in hospice or home health.
• Valid driver’s license and reliable transportation.

• EPIC experience preferred.

Responsibilities

• Serve as the main point of contact for prospective patients and families with questions about home hospice services.
• Explain insurance benefits and service coverage, patient responsibilities, and alternative payment options.
• Coordinate admissions to the appropriate program and complete paperwork promptly and accurately.
• Develop the initial plan of care using input from the patient, family, and physician.
• Provide information and education about Stella Maris Home Hospice services to physicians, discharge planners, and community organizations.
• Assume on-call responsibilities as needed.
