About The Position
Take on a vital role that blends clinical skill with heartfelt connection! James River Home Health is seeking a compassionate and dedicated Home Health Admissions RN to help patients and their families transition into care at home. As our Admissions RN, you'll complete initial assessments, determine eligibility, explain services, and coordinate with the care team to ensure a smooth start of care.
Requirements
- Must be a Registered Nurse with a current license to practice in the Commonwealth of Virginia.
- Proficiency in using electronic medical record (EMR) systems.
Nice To Haves
- At least one year of home health experience is preferred but not required.
Responsibilities
- Complete thorough assessments to determine eligibility.
- Explain our services.
- Guide patients and their families through the admission process with clarity and confidence.
- Work closely with physicians and the interdisciplinary team to gather medical information.
- Ensure a smooth, timely start of care.
- Maintain strong documentation and communication skills.
- Set the tone for high-quality, patient-centered home health care.
