About The Position

We are seeking a compassionate, detail-oriented Admission Nurse (RN or LPN) to join our clinical team. In this role, you will be the first point of clinical contact for incoming patients. You will be responsible for conducting comprehensive initial health assessments, gathering detailed medical histories, and ensuring a seamless, welcoming transition into our care. The ideal candidate thrives in a fast-paced environment and possesses strong clinical judgment, excellent communication skills, and top-tier administrative accuracy.

Requirements

Active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of KY.
Minimum of [1–2] years of clinical nursing experience, preferably in Long Term Care, med-surg, or acute care.
Current Basic Life Support (BLS) certification required.
Proficiency with electronic health record (EHR) systems (e.g., PCC).
Outstanding interpersonal skills with the ability to remain calm and empathetic under pressure.

Responsibilities

Perform Initial Admissions: Conduct head-to-toe clinical assessments, check vital signs, and evaluate immediate patient needs.
Document Medical Histories: Gather and record detailed information regarding patient medications, allergies, past procedures, and advance directives.
Coordinate Care Transitions: Collaborate with physicians, case managers, and floor nurses to develop initial care plans and assign appropriate units.
Manage Patient Intake: Complete all regulatory paperwork, verify consent forms, and ensure compliance with state and federal healthcare standards.
Provide Patient & Family Education: Orient patients and their families to the facility, explain upcoming procedures, and answer questions to alleviate anxiety.
