Hospice of Wichita Falls is seeking compassionate nurses to join their Admissions Team. Requirements

Graduate of an approved school of professional nursing.
Current license by the Texas Board of Nurse Examiners.
Ability to understand the physical, psychosocial, and spiritual concerns/problems of terminally ill patients in a home setting.
Possess critical thinking skills.
Compassionate.
Effective communication both written and orally.

Responsibilities

Introduce Hospice services to patients and families.
Provide patients and families with confidence in their decision regarding hospice care.
Obtain all appropriate documentation for patient admission.
Evaluate patients to establish admission criteria.
Collaborate with the Hospice Physician Team to confirm admission.
Ensure the patient is comfortable and establish the basis for care.
Document care provided to patients.
Ensure team members are documenting care.
Document care in a way that clearly communicates the patient's condition and needs to other team members.
Focus on the patient's greatest need and providing "Care, Hope, and Support for all when Illness threatens life."
Ensure the patient's experience is a positive memory for their loved ones.
