About The Position

The Admissions/Medical Records Nurse (LPN or RN) oversees admissions intake and medical records functions for the Nursing Department(s). The role assesses new residents and reviews baseline care plans with the nursing leadership team. It also provides patient care as needed and participates in the campus on-call rotation according to the campus schedule.

Requirements

• Must have and maintain a current, valid state LPN or RN license.
• Current, valid CPR certification is required.

Responsibilities

• Review Braden Scale results and implement preventative measures for high-risk residents before wound development.
• Review high-risk residents upon admission using a fall risk assessment; ensure orders for preventative interventions are in place, interventions are entered into the care plan, and CNA sheets and profiles are updated.
• Complete nursing admission assessments and baseline care planning for new admissions.
• Complete the MDS Section GG Functional Assessment within the required timeframe.
• Make written and oral reports and recommendations to the Assistant and Director of Nursing as necessary or required concerning the operation of the nursing service department.
• Verify census events are correct in the Electronic Health Record.
• Complete resident documentation in MatrixCare to ensure medical records are complete and accurate.
• Provide patient care as needed.
• Participate in the campus on-call rotation based on the campus schedule.
