About The Position
Abraxas Youth & Family Services is seeking an Admissions LPN to support youth, young adults, and families at Woodridge Intervention. The LPN completes admission nursing assessments, reconciles medications, reviews referral records, and coordinates with healthcare providers, pharmacies, families, admissions staff, and program teams to support safe placement. The role also includes reporting urgent or abnormal findings, educating residents and caregivers, maintaining confidentiality, and providing nursing care or medication administration as assigned.
Requirements
- Current Pennsylvania LPN licensure in good standing.
- Current CPR/First Aid certification.
- Valid driver's license.
Responsibilities
- Complete admission nursing assessments, including vital signs, health history, allergies, symptoms, and immediate health or safety concerns.
- Reconcile medications and verify orders, pharmacy information, last doses, and entries in the electronic MAR.
- Review referral records and obtain missing clinical information, consents, clearances, and provider orders.
- Coordinate with providers, pharmacies, referral sources, families, admissions staff, and program teams to support safe placement.
- Report abnormal findings and urgent concerns to the Nurse Manager and provider and complete required follow-up.
- Educate residents and caregivers on medications, nursing services, follow-up needs, and health procedures.
- Maintain confidentiality and provide nursing care or medication administration as assigned.
