About The Position

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and customer-focused Admissions Coordinator to join our team in West Palm Beach. This position is onsite in a call center environment. This full-time position works Friday through Sunday, 7:00am - 7:30pm OR 9:00 a.m. – 9:30 p.m., supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions. Please note this is a full time position and the training will be Monday - Friday 8am-5pm for 2-3 weeks. Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

Requirements

Associate degree preferred.
Three (3) years of experience in healthcare administration or customer service.
Knowledge of hospice or home health programs preferred, with willingness to develop clinical understanding.
Ability to handle a high call volume.
Strong communication, organization, and computer skills.

Responsibilities

Facilitate the conversion of referrals to admissions through completion of the intake process.
Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
Maintain accurate records and documentation in accordance with organizational policies.
Support a compassionate, patient-centered environment that reflects the mission and values of Trustbridge Hospice.
