Requirements

  • Working knowledge of senior living best practices, including customer service and clinical and financial guidelines.
  • Working knowledge of, and experience with, admissions and discharge procedures.
  • Working knowledge of medical terminology.
  • Must be able to deal respectfully with staff, residents, visitors, and other health care providers.
  • Interpersonal skills.
  • Effective organizational skills.
  • Effective written and verbal communication skills.
  • Knowledge of computer programs and software.
  • A Bachelor’s degree in a related field.
  • Long-term care experience.
  • Must possess a valid driver’s license.

Responsibilities

  • Identify and maintain relationships with referral and inquiry sources. Foster positive, trust-based relationships with residents, their representatives, referrers, and physicians.
  • Responsible for establishing and maintaining open, effective communication between Resthaven and referring publics, consumers, physicians, and third-party payers.
  • Serve as the primary point of contact for potential residents and their families, answering questions and providing information about the facility's services, amenities, and community lifestyle.
  • Coordinate the entire referral process from initial inquiry to move-in, ensuring timely responses and accurate completion of all required paperwork and pre-admission assessments.
  • Receive, research, screen, and evaluate potential residents for appropriate clinical and financial admission. Ensure initial and ongoing fiscal appropriateness of resident admission.
  • In conjunction with the interdisciplinary team, ensure the establishment of a service plan based on prioritized objectives.
  • Participate in internal moves/transfers.
  • Utilize CRM tracking tools to document and analyze referral data to monitor trends, identify opportunities for growth, and assess the effectiveness of referral strategies.
  • Collaborate with the sales and marketing teams to align referral goals with overall business objectives.
  • Address and resolve any issues or concerns that arise during the referral and admissions process to ensure high levels of resident and family satisfaction.
  • Maintain a waiting list for all potential residents.
  • Schedule tours and coordinate meetings between residents, families, and the facility's admissions team.
  • Represent the facility at community events and meetings to build brand awareness and establish the facility as a preferred provider.
  • Assume all other position-related responsibilities as assigned.
