About The Position

Help patients begin their recovery journey with confidence, compassion, and expert care coordination. We are seeking a dedicated and clinically experienced Admissions Coordinator to join our Post-Acute Inpatient Rehabilitation Unit. This role is instrumental in ensuring appropriate patient admissions while delivering an exceptional experience for patients, families, providers, and payers. As a key member of the rehabilitation team, you will evaluate patient eligibility for intensive rehabilitation services, coordinate pre-admission processes, and support seamless transitions into care. This position is ideal for a healthcare professional who thrives in a collaborative environment and is passionate about patient-centered rehabilitation services.

Requirements

  • Current licensure as one of the following: Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech Language Pathologist (SLP)
  • Minimum of two (2) years of clinical experience
  • Current license to practice in the state of Nebraska
  • Strong oral and interpersonal communication skills
  • Ability to effectively manage conflict and navigate difficult conversations
  • Excellent critical thinking and clinical decision-making abilities
  • Knowledge of regulatory agencies, standards, and compliance processes
  • Strong organizational skills with the ability to manage multiple priorities simultaneously
  • Commitment to compassionate, patient-centered care

Nice To Haves

  • Experience working in an Inpatient Rehabilitation Unit
  • Certification in Rehabilitation

Responsibilities

  • Coordinate the pre-admission process for the inpatient rehabilitation unit
  • Conduct clinical assessments to determine patient appropriateness for intensive rehabilitation services
  • Collaborate with physicians, nurses, therapists, care coordination teams, and insurance providers
  • Evaluate patients’ medical needs, therapy tolerance, nursing requirements, and discharge planning potential
  • Obtain prior authorizations and communicate financial responsibilities as needed
  • Educate patients and families regarding rehabilitation programs, expectations, and routines
  • Maintain accurate, concise, and timely clinical documentation
  • Support utilization management activities within post-acute care
  • Complete orientation and credentialing for the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI)
  • Adhere to all organizational safety standards, policies, procedures, and regulatory requirements
  • Effectively prioritize workload and utilize resources responsibly
