About The Position

This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities. The Hospice Admission Coordinator, Social Worker will work with the hospital, home health, other facilities (hospitals, ALF, SNF) and hospice team to coordinate care across the continuum. The hours are mostly daylight with occasional evenings and weekends as required to meet patient and family needs. Hospital consults and facilitation of patients coming onto hospice will be the primary focus of the role, however, there may be a need for home-based (patient home) visits.

Requirements

  • Bachelor of Science in Social Work
  • BLS
  • 2 years of social work experience in case work or counseling in a health care or social services delivery system
  • 1 year of social work experience in palliative care or hospice
  • Driver's License: Yes
  • To drive an Augusta Health Vehicle, must be 21 years old and Motor Vehicle Record (MVR) must be approved by Risk Management.
  • Eligibility to work in the United States and meet Virginia state employment requirements

Nice To Haves

Responsibilities

  • Possesses a working knowledge of social work principles and hospice care.
  • Demonstrates clinical competence and consistently makes appropriate clinical decisions.

  • Coordinates hospice referrals, admissions, and transitions of care.
  • Consults with hospital staff regarding current and prospective hospice patients.
  • Receives referrals from hospital staff and physicians.
  • Communicates and collaborates with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
  • Assesses patients for appropriateness of referral to hospice and provides care for patients across the life spectrum.
  • Assesses hospice patients to identify physical, psychosocial, and environmental needs.
  • Informs patients and their support systems of hospice benefits and the responsibilities of the patient and caregiver.
  • Recommends appropriate patient placement upon discharge.
  • Assists with coordination of care, hospice DME, orders, and medications when transitioning to hospice from an acute hospital stay or Emergency Department visit.
  • Coordinates financial assistance for patients in need with hospital social workers.
  • Communicates and documents patient care information accurately and timely.
  • Conveys accurate information and relays it in a timely manner to the hospice team and hospital team involved in the patient’s care.
  • Accurately receives information and relays it in a timely manner.
  • Provides effective communication to patients, family members, team members, and other health care professionals.
  • Completes documentation of all interactions in the patient’s hospice record, ensuring information is accurate and appropriate.
  • Ensures timely submission of all required paperwork in accordance with policy.
  • Educates hospital staff regarding the benefits of hospice care.
  • May be required to complete social work visits for home team patients on an as needed basis.
  • Provides care utilizing appropriate infection control measures and in accordance with applicable OSHA requirements.
  • Demonstrates commitment to professional growth by participating in hospice-sponsored in-service programs and maintaining or improving competency.
  • Participates in the Hospice QAPI program.
  • Promotes the Hospice philosophy.
