We are currently seeking a compassionate, dedicated Registered Nurse (RN) to join our Admissions Team Monday-Friday 9pm-5pm. Requirements

Registered Nurse (RN) with a valid, active license in Pennsylvania.
Valid driver’s license required.
Excellent communication skills (both verbal and written) with the ability to educate and guide patients and families.
Strong problem-solving skills, critical thinking and attention to detail.
Ability to perform physical tasks such as lifting, transferring patients, and reaching, bending, kneeling, and standing for extended periods.
Basic math skills for medication and dosage calculations.
A compassionate and empathetic approach to patient care.

Nice To Haves

Minimum of 1-2 years of recent hospice experience preferred, but candidates with strong nursing backgrounds will be considered.

Responsibilities

Assessing patient eligibility for hospice care and explaining the hospice benefit to patients and families.
Managing immediate symptoms and providing education to patients and families to ensure comfort and understanding.
Collaborating with the attending physician and hospice medical director to develop and implement the initial plan of care.
Providing skilled nursing care with a focus on pain control and symptom management.
Explaining hospice services and the Medicare benefits clearly and compassionately to patients, family members, and significant others.
Ensuring all initial patient and family needs are met with empathy and professionalism.
Collaborating with the Interdisciplinary Team (IDT) to address the patient's medical, emotional, and spiritual needs.
Building and maintaining relationships with long-term care facilities, hospital staff, and physicians to promote and expand hospice services.
Offering guidance on the patient’s diagnosis, medications, and treatments.
