We are currently seeking a compassionate, dedicated Registered Nurse (RN) to join our Admissions Team Monday-Friday 9pm-5pm. Requirements

  • Registered Nurse (RN) with a valid, active license in Pennsylvania.
  • Valid driver’s license required.
  • Excellent communication skills (both verbal and written) with the ability to educate and guide patients and families.
  • Strong problem-solving skills, critical thinking and attention to detail.
  • Ability to perform physical tasks such as lifting, transferring patients, and reaching, bending, kneeling, and standing for extended periods.
  • Basic math skills for medication and dosage calculations.
  • A compassionate and empathetic approach to patient care.

Nice To Haves

  • Minimum of 1-2 years of recent hospice experience preferred, but candidates with strong nursing backgrounds will be considered.

Responsibilities

  • Assessing patient eligibility for hospice care and explaining the hospice benefit to patients and families.
  • Managing immediate symptoms and providing education to patients and families to ensure comfort and understanding.
  • Collaborating with the attending physician and hospice medical director to develop and implement the initial plan of care.
  • Providing skilled nursing care with a focus on pain control and symptom management.
  • Explaining hospice services and the Medicare benefits clearly and compassionately to patients, family members, and significant others.
  • Ensuring all initial patient and family needs are met with empathy and professionalism.
  • Collaborating with the Interdisciplinary Team (IDT) to address the patient's medical, emotional, and spiritual needs.
  • Building and maintaining relationships with long-term care facilities, hospital staff, and physicians to promote and expand hospice services.
  • Offering guidance on the patient’s diagnosis, medications, and treatments.
