About The Position

The Osborn is seeking a compassionate and detail-oriented Admission Registered Nurse (RN) to join our team on a per diem basis. This role is essential in overseeing new resident admissions, managing intake, and ensuring a smooth transition into our community. The Admission RN will conduct clinical assessments, provide direct nursing care, and coordinate with interdisciplinary teams to deliver exceptional resident-centered care. We are looking for a professional RN who demonstrates strong clinical judgment, organizational skills, and a commitment to excellence in long-term care.

Requirements

Active NY State RN license and current CPR certification required.
Strong clinical assessment, documentation, and organizational skills.
Excellent communication and interpersonal abilities.
Ability to manage multiple priorities and work effectively as part of a team.
Knowledge of regulatory requirements for long-term care facilities.

Nice To Haves

Prior experience in admissions, long-term care, or skilled nursing preferred.

Responsibilities

Coordinate and oversee all aspects of new resident admissions and intake.
Conduct thorough nursing assessments and initiate individualized care plans.
Provide direct nursing care to residents as needed, ensuring safety and comfort.
Supervise and delegate tasks to Nursing Assistants in compliance with facility standards.
Accurately document resident assessments, care plans, and progress notes.
Administer medications and treatments as prescribed by healthcare providers.
Monitor residents’ conditions and promptly report changes to appropriate staff.
Ensure compliance with infection control and safety protocols.
Collaborate with interdisciplinary team members to support residents’ health and well-being.
Promote professionalism and uphold the mission, vision, and values of The Osborn.
