Requirements

  • Graduate of an accredited nursing program
  • Current, unrestricted Registered Nurse (RN) license in the state of practice
  • Minimum one (1) year of Home Health experience
  • Strong OASIS knowledge and documentation experience
  • Valid driver's license, reliable transportation, and current auto insurance
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Strong clinical assessment, communication, and organizational skills

Responsibilities

  • Serve as the first clinical point of contact for patients and families, ensuring a smooth transition into home health services while delivering exceptional clinical assessments and support.
  • Conduct comprehensive admission assessments to identify patients' physical, psychosocial, and environmental needs.
  • Complete OASIS assessments accurately and timely.
  • Develop and implement individualized plans of care that support positive patient outcomes.
  • Perform initial nursing evaluations and ongoing reassessments as appropriate.
  • Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care.
  • Communicate significant patient condition changes and clinical findings promptly.
  • Ensure accurate, timely, and compliant documentation in the medical record.
  • Maintain patient privacy and confidentiality in accordance with HIPAA regulations.
  • Identify and report safety concerns and implement appropriate interventions.
  • Support quality care initiatives and organizational standards.
  • Perform additional duties and projects as assigned.
