Requirements

Graduate of an accredited nursing program
Current, unrestricted Registered Nurse (RN) license in the state of practice
Minimum one (1) year of Home Health experience
Strong OASIS knowledge and documentation experience
Valid driver's license, reliable transportation, and current auto insurance
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Strong clinical assessment, communication, and organizational skills

Responsibilities

Serve as the first clinical point of contact for patients and families, ensuring a smooth transition into home health services while delivering exceptional clinical assessments and support.
Conduct comprehensive admission assessments to identify patients' physical, psychosocial, and environmental needs.
Complete OASIS assessments accurately and timely.
Develop and implement individualized plans of care that support positive patient outcomes.
Perform initial nursing evaluations and ongoing reassessments as appropriate.
Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care.
Communicate significant patient condition changes and clinical findings promptly.
Ensure accurate, timely, and compliant documentation in the medical record.
Maintain patient privacy and confidentiality in accordance with HIPAA regulations.
Identify and report safety concerns and implement appropriate interventions.
Support quality care initiatives and organizational standards.
Perform additional duties and projects as assigned.
