About The Position

The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient’s charts for accuracy and will QA all necessary forms needed for admissions. He/she will follow-up with patients on the NTUC list & provide additional information about hospice services. Responsible for answering incoming telephone calls from patients/caregivers/medical professionals. Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications. Proficient in receiving, obtaining and relaying accurate information to be disseminated. Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions. May be required to visit patients when needed. Provides care within scope of practice.

Requirements

  • Current Florida LPN license required.
  • Current CPR certification.
  • Minimum of three years of nursing experience in a home health/hospice setting within the past five years is required.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be self-directed and able to work independently or as a team.
  • Basic computer skills, use of common office software and EMR software experience.
  • Excellent people and relationship-building skills.
  • Excellent customer service skills.
  • Maintain effective work relationships with co-workers, the public and administration.
  • Highly organized, adaptable problem solver and ability to work under stress or pressure.

Responsibilities

  • Understanding of and ability to execute routine Hospice Nursing: crisis intervention, communication skills, teaching skills, thanatology skills and needs.
  • Under the supervision of the RN Admissions, when needed, provide skilled professional nursing care to the patient and family unit incorporating physical, psychosocial, spiritual, cultural, and biological components.
  • Utilizing the nursing process, demonstrating accurate data collection, and assisting in the planning, implementation and delivery of clinical care services.
  • Implementing the nursing plan of care in accordance to the physician orders and under the RN Case Manager.
  • Providing appropriate pain and symptom management, including the patient’s response to treatment and medications.
  • Documenting patient care which reflects nursing intervention, patient's response to care and progress towards goals as set in the Plan of Care.
  • Maintaining clerical type records via the computer or handwritten data.
  • Demonstrating ability to provide an effective, supportive, sensitive approach in response to needs of patients, families and staff.
  • Being polite and courteous at all times.
  • Providing program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.
  • Working collaboratively with other departments, demonstrating and encouraging leadership for a comprehensive approach to program development.
