About The Position
Join our growing team as an admissions coordinator! This full-time position (40 hours/week) will be based at our Eau Claire – West location, serving multiple Dove Healthcare locations. As an admissions coordinator, you will be responsible for coordinating admissions with hospital discharge planners and other referral sources, assessing and coordinating a smooth admission transition, confirming necessary medical and financial information, developing and maintaining strong relationships with referral sources, and ensuring patient continuity of care.
Requirements
- Professional health care experience required (social worker, nurse CMA, CNA, pharmacy tech, PTA, COTA, etc.)
- Ability to multitask, juggle multiple priorities and work in a fast-paced environment
- Proficiency in relevant software tools / applications such as Microsoft.
- Detailed oriented with strong organizational and time management skills
- Strong problem-solving abilities
- Proficiency in written and verbal communications
Nice To Haves
- Experience with EPIC/Care Anywhere, Point Click Care (CRM) is helpful.
