About The Position

Responsible for ensuring prompt and efficient bed management, assist with staff resource utilization and Referral Line Management. Serves as a liaison to outside admitting physicians, outside facilities and the Emergency Department for patient referrals and direct admissions.

Requirements

  • Associates Degree in Nursing.
  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Three (3) years clinical experience.
  • Ability to be scheduled for work based on the operational needs of the hospital.
  • Possess the ability to deal tactfully and harmoniously with patients, family members, guests, administrative staff and physicians.
  • Ability to organize and prioritize time and tasks to achieve a well-coordinated work effort and to efficiently meet work schedules including an ability to integrate multiple factors which may have an impact on patient care, including variance in human resources.
  • Maintains appropriate certifications: BLS, ACLS
  • Access patient transport tele-tracking accurately and appropriately.
  • Wears and utilizes the appropriate communication devices for communications between co-workers, referring physicians/facilities, and other members of the healthcare team.
  • Documentation and signatures are legible, concise and complete.

Responsibilities

  • Facilitate suitable and timely bed placement for patients by problem solving with appropriate departments, administrative personnel, and House Supervisor. Serves as the primary contact in facilitating direct admissions from referring physicians and outside facilities through management of the Direct Admissions/Referral Line. Facilitates three way phone calls between outside physicians/facilities and admitting/accepting CCMC physicians and Emergency Department.
  • Oversees all patient placement activity during designated shift and plans for anticipated activity for other shifts. Communicates bed availability and patient placement prioritization plan to all areas. Consistently maintains the Bed Board throughout their shift to facilitate timely and accurate bed availability. Collaborates with appropriate House Supervisor to facilitate timely bed placement.
  • Acts as a resource and facilitator to nursing and the Admitting Department staff to expedite the bed assignment process.
  • Attend/facilitate Bed Huddles to identify and organize patient placement plan.
  • Serve as a central problem solver for staffing office, admitting, attending physicians, nurse managers, charge nurses, team leaders, and front line staff for all patient care, staffing and patient placement concerns.
  • Monitor occupancy levels and consults with Nursing Administration and House Supervisors to evaluate the initiation of the High Census Plan.
  • Coordinates with the Nursing Staffing Office regarding any staffing issues or needs. Collaborates with the House Supervisors to resolve any issues.
  • Coordinate the best placement of patient to meets their individual needs.
  • Facilitate bed assignment process to decrease patient wait time and prevent bed boarding in the ED or other holding areas.
  • Monitors Bed Board and notify nursing units to discharge patients from the computer system in a timely manner as appropriate.
  • Participate in the interpretation, development, implementation, and ongoing evaluation of policies and procedures related to patient placement, bed management and staffing activities.
  • Recommend and facilitate improvements in the patient placement process.
  • Management decisions reflect integration of the West Virginia Nurse Virginia Nurse Practice Act, Collaborative Practice Model, The Joint Commission standards and other relevant standards of care/practice.
  • Demonstrates appropriate patient / family / self-safety practices.
  • Oversees, advises, participates and/or finds appropriate resources for patient care activity as identified per shift to facilitate safe care.
  • In conjunction with the Manager-Nursing, and House Supervisors participates in the coordination and the allocation of available resources, in assigning nursing staff according to established staffing and scheduling practices to meet patient care hour requirements.
  • Recognizes the need for and supports performance management (PM), facilitates implementation of PM findings on unit, and actively participates in PM activities.
  • Recognizes poor work performance and noncompliance to hospital policies and procedures, conducts management/staff conferences, assists staff with performance improvement and compliance per Human Resources procedures and provides accurate documentation of such interactions.
