This is a Non-Clinical Admission Role.

Requirements

• Minimum of three years in the medical field
• High school graduate or equivalent
• Must be able to lift up to 50 lbs
• Ability to walk, climb stairs, push and/or pull equipment and residents as needed
• Prolonged standing or walking
• Able to maintain high energy level throughout an eight hour shift

Nice To Haves

• Basic understanding of insurance, electronic medical records and medical terminology preferred
• General use of Microsoft Office Tools
• Genuine interest in geriatrics and assuring quality medical records of residents
• Ability to adapt to frequent changes in routine
• Prefer LPN, RN, MSW, but college courses would be acceptable

Responsibilities

• Review all referrals submitted by hospitals for admission to rehab and/or LTC
• Open communication with hospital discharge planners on acceptance or denial of patient referrals
• Ensure required referral information and data are obtained timely prior to an admission arriving
• Entering patients in the EMR
• Follow up daily on all pending referrals
• Completion admission paperwork
• Bridge communication with the team regarding new admissions
