About The Position

Constellation Health Services is seeking an Administrator - RN for our Certified Home Health Program. The Administrator assumes management responsibility for the fiscal and operational activities of the home health program. They are responsible for long-range planning, organizing, directing, and staffing of the organization. The Administrator plans and develops new programs, recruits, and interviews management staff, and establishes mechanisms for measuring quality care and organizational performance. Requirements

  • Registered Nurse (RN) in the state of employment.
  • They have the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state.
  • Clinical leadership and operations experience in Medicare Certified Home Health.
  • Expert knowledge in Home Health regulations and OASIS.
  • Eight years of certified home health experience, five of which were in management/leadership.

Responsibilities

  • Oversee the development of organizational policies and procedures that support the mission and goals of the organization while ensuring credentialing and regulatory compliance.
  • Provides leadership and consultation to the management team.
  • Holds regular management team and staff meetings to ensure smooth operations and communication flow throughout the organization.
  • Provides procedures and a system of staff communication that ensures coordinated implementation of Plans of Treatment and utilization of necessary services to be able to give quality care to our patients.
  • Develops effective strategic goals and operational/implementation plans for Allocates and delegates work effectively and works to help employees achieve maximum potential.
  • Interfaces and communicates with DHS, CMS. and other Federal, State and Private organizations on behalf of the organization.
  • Ensure development and implementation of record systems and statistical reporting systems for program documentation, planning and evaluation and ensure that all required records are current.
  • Demonstrates positive written and verbal communications, internal and external to the organization.
  • Demonstrates a high commitment to customer services and quality organizational standards.
