As the Administrative Assistant, you will serve as a trusted resource of the interdisciplinary team to support patient admissions, intake coordination, medical records management, and day-to-day office operations. Your role is essential to ensuring patients and families receive timely, compassionate service throughout their care journey.

Requirements

  • High school diploma or equivalent required.
  • Previous Home Health or Hospice intake experience required.
  • Strong communication, customer service, and relationship-building skills.
  • Ability to multitask, prioritize responsibilities, and remain organized in a fast-paced environment.
  • Professional, positive, and compassionate approach when interacting with patients, families, and staff.
  • Flexible, adaptable, and able to manage changing priorities.

Responsibilities

  • Serve as the primary point of contact for patients, families, referral partners, physicians, and care team members, delivering compassionate and professional service.
  • Provide administrative support including answering phones, scheduling, managing correspondence, and data entry.
  • Coordinate and manage patient referrals from receipt through admission.
  • Verify patient demographics, insurance information, and physician orders to ensure accurate admissions.
  • Support intake and clinical teams during periods of increased referral volume and census growth.
  • Maintain patient medical records and charts in compliance with agency policies and regulatory requirements.
  • Prepare, scan, upload, and organize documentation within electronic medical records.
  • Track intake activity, census reports, and key operational metrics.
  • Monitor clinical and office supply inventory, coordinating orders to ensure uninterrupted patient care.
  • Assemble admission packets and ensure all required documentation is accurate, complete, and organized.
  • Create and coordinate patient welcome gift baskets that reflect our commitment to comfort and support.
  • Maintain inventory levels of admission materials and gift basket supplies to ensure readiness for new admissions.
  • Support special projects and operational initiatives assigned by the Director of Operations.
