Provide best practice assessment, treatment, navigation and support to individuals residing within a Home First transitional residential community program, with a primary diagnosis of substance use disorder. This role involves working in collaboration with an interdisciplinary team to enhance client's skills and abilities to live independently following discharge from a transitional housing program. The worker will liaise with various psychiatric, medical, and community agencies to ensure continuity of care and support service needs. Key duties include completing assessments, developing and implementing treatment plans, documenting progress, and facilitating education and group treatment sessions using strength-based, therapeutic techniques such as harm reduction and motivational counselling.
