About The Position

The Addiction Specialist Coordinator supports individuals with Substance Use Disorders (SUD), including opioid and methamphetamine use, through comprehensive care coordination and engagement with Medication-Assisted Treatment (MAT) services. The Coordinator strengthens connections between hospital and emergency department (ED) discharges, outpatient care, home health, nursing, psychiatry, and case management to promote continuity and stabilization. This position plays a central role in implementing and supporting low-barrier access to treatment and will help launch and maintain a Contingency Management (CM) program within supportive housing. This evidence-based approach provides tangible incentives to encourage recovery milestones such as treatment participation and drug abstinence. The position also develops and implements care plans, coordinates services and resources, monitors client progress, provides advocacy and support, facilitates referrals to other services, and maintains case documentation and data collection. Embedded within the SIPPRA (Social Impact Partnerships to Pay for Results Act) team, the Coordinator contributes to integrated, outcome-driven approaches that advance recovery and housing stability.

Requirements

High School Diploma or GED required.
At least 2 years’ experience in addiction services, case management, or healthcare coordination preferred.
Familiarity with MAT, recovery focused interventions, and trauma-informed care models strongly preferred.
Experience working with people experiencing homelessness or behavioral health challenges highly desirable.
Bilingual or multilingual skills preferred.
Strong understanding of recovery focused interventions, motivational interviewing, and trauma-informed principles.
Excellent interpersonal and communication skills with the ability to build rapport and trust.
Skill in organization, multitasking, and maintaining attention to detail in a fast-paced setting.
Proficiency in electronic health record systems and Microsoft Office Suite.
Ability to manage confidential information with integrity and professionalism.
Capacity to work collaboratively within a multidisciplinary team and across systems.
Comfort in de-escalating crises and supporting clients through behavioral health challenges.
Must have and maintain in good standing, a Colorado driver’s license within 30 days of hire (with the exception of SSHC). This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business.

Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred

Responsibilities

Coordinate comprehensive care for clients engaged in Substance Use Disorder (SUD) treatment, ensuring integration across behavioral health, medical, psychiatric, and case management services.
Facilitate access and continuity of care by coordinating referrals, transitions from hospitals or detox programs, and re-engagement following missed appointments or lapses in treatment.
Support medication-assisted treatment (MAT) by collaborating with prescribers, nurses, and care teams to promote adherence, timely follow-up, and consistent client engagement.
Implement and monitor Contingency Management (CM) initiatives to support recovery for individuals with stimulant and opioid use disorders, including tracking participation and documenting outcomes.
Provide client-centered care management focused on stabilization, recovery, housing retention, and connection to long-term supports and benefits.
Promote client safety and crisis stabilization through proactive engagement, crisis intervention, and coordination of emergency or behavioral health services as needed.
Collaborate across disciplines and with community partners to enhance coordination, address systemic barriers, and improve access to services.
Maintain accurate and timely documentation in the Electronic Health Record (EHR) to support care continuity and compliance with organizational and regulatory standards.
Contribute to a culture of quality improvement by identifying service gaps, sharing best practices, and participating in team learning and development.
Ensure confidentiality and uphold professional and ethical standards in all interactions and documentation.
Ensure timely & accurate use of all company systems as required while maintaining complete up-to-date records and any future upgrades as directed.
