Addiction and Mental Health Worker, HART Hub
About The Position
The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access a safe place to rest, obtain basic needs, health care, peer supports, employment and income supports, mental health and addiction services, and other services under one location. Individual units allow opportunities for autonomy and independence while accompanying support services ensure basic and personal needs are met while the individual works towards treatment, recovery and wellness. Addiction and Mental Health Workers within the Hubs program, will perform duties and responsibilities consistent with assisting clients with integrated care planning, system connection and navigation, onsite support with basic needs, mental health and addiction supports. Requirements
- Completed post-secondary education in a mental health or addictions related field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Valid Driver’s License: minimum G2 class required
- Transportation of clients in personal vehicle required; two million-dollar ($2 000 000.00) liability insurance required
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
Nice To Haves
- Current certification in Non-Violent Crisis Intervention (NVCI), ASIST Suicide Intervention, and/or Standard First Aid
- Experience working with individuals with addictions or problematic substance use
- Experience working with individuals with no fixed address
- Residential/community housing experience and familiarity with the Residential Tenancies Act
- Experience in crisis intervention/management and crisis resolution

Responsibilities
- Provide intake, screening, assessment and treatment planning
- Support clients on their journey towards physical/mental health improvement
- Ensure clients basic needs are being met (e.g., provision of food, showers, hygiene and a place to rest)
- Provide referrals to other supports and resources to maintain overall wellness
- Assist with the development of care plans, navigating local services, and engaging in proactive crisis intervention
- Provide one-on-one and group counselling sessions with a strengths-based wellness approach
- Ensure client record is updated with changes in circumstances, housing, medications, health conditions, diagnoses, etc.
- Provide navigation assistance, connection to resources, and collaboration with established justice supports for those involved with the criminal justice system (pre-release support, probation or bail)
- Provide accessible options for communication for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation and referral to language classes)
- Assist with transportation needs upon intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support
- Provide integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation and safety
- Provide guidance with income planning, applications (e.g., ODSP, OW, CPP), and tax filing
- Provide connections to employment support programs
- Provide strategies to access barriers, build confidence and improve employability
- Provide 24/7, on-site support to help individuals maintain stable housing at HART Hub
- Facilitate ongoing conversations around housing goals to secure suitable housing
- Provide rapid connection to housing stability resources
Benefits
- Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated)
- Wage differential for regulation for successful candidates registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO)
- Grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
- Premiums of $2.00 per hour will be applied for eligible Evening/Night (8:00pm – 7:00am) and Weekend Shifts.
- Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)
- 9% in lieu of benefits
