A full-time non-exempt position with primary working hours of Monday through Friday 8:30am-4:30pm (37.5 hours/week). Guaranteed hourly rate increase at 12 months of service. Paid training provided. An overview of the responsibilities of the Acute TCM position include: Responsible for assessing client functioning and developing comprehensive Targeted Case Management (TCM) plans for individuals transitioning from psychiatric facilities. Coordinates discharge planning with hospital liaisons, links clients to housing, benefits, and treatment services, and ensures intensive monitoring during the first 90 days post-discharge. Oversees all services received by clients, ensures needs are met through natural and community supports, attends interdisciplinary meetings, and maintains a current directory of resources. Participates in a rotating 24-hour crisis response system, provides immediate support, and ensures all alternatives to hospitalization are explored. Acts as a strong advocate for clients, removing barriers, resolving issues, and ensuring access to appropriate services and resources. Maintains accurate records, ensures regulatory compliance, prepares for audits, and demonstrates strong knowledge of case management principles and community resources.
