Coordinates and provides specialized nursing care to patients receiving acute dialysis treatments in hospital and contracted hospitals including, but not limited to, hemodialysis, peritoneal dialysis, and ultrafiltration. Responsible for providing safe, complete, and effective dialysis treatments including hemodialysis, peritoneal dialysis, and ultrafiltration for inpatient populations in the hospital or contracted hospital with limited supervision and in accordance with physician orders, policies, and procedures. Proficient in assessment, cannulation and management of dialysis access including AVF, AVG, Buttonhole, and CVC, with the ability to identify and respond to access related complications. Responsible for direct patient care of assigned patients in the hospital or contracted hospital setting, including pre, intra, and post treatment assessment, and providing the appropriate treatment technique for assigned patients as prescribed in compliance with all modality procedures, including hemodialysis, peritoneal dialysis, and ultrafiltration. Performs comprehensive ongoing assessment of patients’ responses to treatment and makes appropriate adjustments and modifications to the treatment plan as ordered by the Nephrologist and within the scope of practice. Communicates timely and clinically relevant treatment concerns, complications, or changes in patient condition to the Nephrologist and appropriate members of the care team. Collaborates and communicates with physicians/Nephrologist and other members of the care team to assess, adjust, and ensure safe, coordinated, and continuous patient care across the inpatient setting. Maintains accurate, complete, and timely patient records; utilizes electronic treatment recording platforms and hospital EMR systems; troubleshoots dialysis related equipment; and adheres to infection prevention, sterile technique, and safety standards during all procedures. Provide on-call coverage for hospital or contracted hospital dialysis treatments as scheduled, including response to urgent of emergent dialysis needs. Demonstrates strong assessment, critical thinking, and problem-solving skills related to acute dialysis treatments, including the ability to recognize and respond to hemodynamic instability and dialysis related emergencies. Coordinates and administers medical and nursing care to patients utilizing the nursing process as a framework for practice, prioritizing patient safety, clinical acuity, and physician directed care plans. Administers medications, IV therapies, and treatments in a timely manner, safely and in accordance with hospital policy, dialysis protocols, and physician orders. Provides patient can caregiver education, as appropriate for the inpatient setting, using methods tailored to varying health literacy levels and clinical conditions. Demonstrates the ability to function independently while managing inpatient acute dialysis assignments, including prioritization of multiple patients, collaboration with hospital staff, and escalation of care when indicated. Demonstrates proficiency with electronic medical records, dialysis monitoring systems, and related technology to support accurate documentation, patient safety, and regulatory compliance.
