About The Position

Deaconess Cardiology is seeking an Acute Care Nurse Practitioner or Physician Assistant or Family Nurse Practitioner with Cardiology experience to join the Structural Heart department. This role involves split duties between inpatient and outpatient settings. The provider will offer consultation and care for hospital patients requiring structural heart consults, including pre and post-procedural care for TAVR and other structural heart procedures. Responsibilities include assisting during TAVR procedures, completing post-procedure discharges, performing H&Ps, patient education, ensuring consent is obtained, conducting post-procedure assessments, placing orders, patient rounding, and managing acute structural heart patients admitted to the hospital. The role also requires participation in the multidisciplinary TAVR conference, contributing documentation and presenting pertinent testing and procedure information weekly. The provider will engage in shared decision-making focused on patient-centered care, adhering to ACC guidelines. Additionally, the position includes managing a part-time outpatient schedule for hospital discharge and follow-up patients, seeing patients at 1-month post-op and 1-year follow-up appointments, and managing patients awaiting surgical intervention.

Requirements

  • Licensed in the State of Indiana
  • Certified by the AANP as a Nurse Practitioner or certified by the NCCPA as a Physician Assistant
  • Cardiology experience

Responsibilities

  • Provide consultation and care for hospital patients as structural heart consults orders are placed, including pre and post-procedural care associated with TAVR and other structural heart procedures.
  • Assist during TAVR procedures and complete post-procedure discharges.
  • Completing H&Ps as needed, completing patient education prior to procedure, ensuring consent is obtained and in the chart, assessments post procedure, placement of post procedure orders, patient rounding/discharge, working up any acute SH patients who have been admitted to the hospital.
  • Participate in the multidisciplinary TAVR conference, contributing to and completing all required documentation.
  • Weekly PowerPoint with pertinent testing and procedure information for required multidisciplinary conference.
  • Engage in shared decision-making processes with a focus on patient-centered care, as outlined in the latest ACC guidelines.
  • Manage part time outpatient schedule to see hospital discharge and follow up patients as outlined in the registry.
  • Seeing patients 1 month post Op and 1 year as needed, follow- up appointment for patients being managed until surgical intervention is needed.
