The inpatient pharmacy at BUMCT provides medications to patients in all areas of the hospital and several hospital-based clinics. We dispense approximately three million doses of medication annually and service a very acutely ill population while providing an elevated level of pharmacy services to our patients 24 hours a day, 7 days a week. This is an Acute Care Clinical Pharmacist Specialist, Pediatric ICU position. You will primarily be Pediatric ICU unit-based with decentralized clinical services such as order verification, medication review, pharmacokinetic monitoring, renal dosing, TPN management, etc..) You may have some responsibility to cover pediatric satellite/centralized staffing on occasion. We strive to ensure our team members understand the full vision of our customer service model and want each Pharmacist to be comfortable in multiple roles within our Pharmacy department. In this role, you will have the opportunity to precept pharmacy residents and students where you will be responsible for their oversight and providing timely evaluations. Pediatric ICU experience is strongly preferred. The schedule for this position is dayshift Monday - Friday with rotating weekends and holiday coverage. POSITION SUMMARY This position actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity. This position is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Conducts educational activities, applies principles of performance improvement, and participates in Pharmacy and Therapeutics Committee/Clinical Consensus Group activities as appropriate. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients being served. Accountable for departmental, and individual patient outcomes by assessing and evaluating data reflective of the patient’s status, interpreting the appropriate information needed to identify each patient’s requirements, and providing pharmaceutical care based on individual needs. Responsible for the delivery of pharmaceutical care to patients using the medication use process. Oversees the activities of the pharmacy technicians, coordinating workflow, and the checking of medications prior to being dispensed. Adheres to local, state, and federal laws regarding the profession of pharmacy practice.
