About The Position

Physicians Southwestern is seeking an Acute Care Advanced Care Practitioner for ArchWell Health. This role involves rendering outpatient primary care medical and consulting services, focusing on acute care needs such as walk-in appointments, sick visits, and hospital follow-ups. The practitioner will work collaboratively with care team members to ensure high-quality patient care and a positive patient experience, minimizing wait times and addressing urgent needs promptly. Responsibilities include maintaining accurate patient records, participating in clinical meetings, and potentially being available for call coverage. The role also involves community outreach and promoting the ArchWell Health model of care. The ideal candidate will be comfortable managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions and possess strong interpersonal skills.

Requirements

  • Currently licensed to practice professional nursing in the state of employment, active and non-probationary.
  • Experienced in primary care: internal medicine, geriatrics, or family medicine, with a minimum of one year of relevant work experience.
  • Current BLS/ACLS certification.
  • Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions.
  • Strong understanding of examination methodologies and diagnostics.
  • Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members.
  • Confident in their ability to deliver high-quality primary care, including being available to patients and addressing their medical and social needs.
  • Passionate about their work and energized by being part of a fast-growing organization.
  • Comfortable traveling within the market to multiple centers within short distance of each other.

Responsibilities

  • Rendering outpatient primary care medical and consulting services for ArchWell Health, pursuant to physician supervision and collaboration agreements.
  • Keeping and maintaining appropriate records of all professional services.
  • Appropriately documenting the care of patients, ensuring accuracy of patient records, encounter notes, medical diagnoses, and documentation.
  • Working with care team members and other employees in a collaborative manner to provide high-quality care and maintain excellent morale.
  • Working with colleagues to ensure a high-quality patient experience, minimizing wait times and allowing timely evaluation of urgent needs.
  • Participating in public relations, direct sales, or outreach events to engage the community and promote the ArchWell Health model of care.
  • Participating in clinical meetings, including reviews of hospitalized patients, clinical outcomes, quality improvement, and peer review activities.
  • Being available for call coverage on nights, weekends, and/or holidays.
  • Rendering primary care and consulting services with a focus on acute care needs, including walk-in appointments, sick visits, and hospital follow-ups.
  • Supporting and partnering with RNs and other clinical and operations colleagues to provide appropriate, timely care.
  • Communicating and documenting the plan of care with members’ Primary Care Provider to maintain strong documentation and continuity of care.
  • Partnering with operations and medical leadership to identify members needing acute support, outreach to members, and perform appropriate services.
  • Partnering with Medical Assistants and other members of the ArchWell Health care team to provide high-quality care.
