About The Position

Plum City Care Center is seeking a Part-Time Activity Assistant to help plan and organize daily activities for residents under the supervision of the Activity Director. This role involves assessing resident needs, implementing activities, and ensuring a safe and engaging environment.

Requirements

  • Ability to gather data from residents for activities assessments.
  • Ability to communicate resident information to the Activities Director and other caregivers.
  • Ability to plan and carry out individual and group activities.
  • Ability to adapt activities to resident's cognitive and physical functional levels.
  • Ability to provide transportation to and from Activity Areas.
  • Ability to monitor and report safety, housekeeping, and maintenance issues.
  • Ability to maintain a clean and orderly activities space.
  • Ability to perform additional cleaning tasks as assigned.

Responsibilities

  • Gathers data from assigned residents to complete and update activities assessments.
  • Communicates resident information to the Activities Director.
  • Identifies ways to support resident's individual needs and preferences, customary routines, concerns and choices.
  • Carries out individual and group activities that meet the functional levels, needs and interests of each resident.
  • Provides activities to bed-bound residents and other residents confined to their rooms that reflect life-long interests.
  • Assists residents with activities as needed.
  • Provides transportation to and from Activity Areas.
  • Adapts activities to match the cognitive and physical functional levels of the residents.
  • Assists Director of Activities in developing activity calendars.
  • Posts calendars on resident halls and distributes these schedules to residents.
  • Monitors physical environment for safety or fire hazards and corrects or reports potential problems immediately.
  • Promptly reports housekeeping and maintenance problems.
  • Replaces all activities equipment and supplies in the proper storage areas.
  • Maintains activities space in a clean and orderly manner that appears "homelike" and is safe for the residents.
  • Performs additional cleaning tasks as assigned by Director of Activities.
