About The Position

Plum City Care Center is seeking a Part-Time Activity Assistant to help plan and organize daily activities for residents under the supervision of the Activity Director. This role involves assessing resident needs, implementing activities, and ensuring a safe and engaging environment.

Requirements

Ability to gather data from residents for activities assessments.
Ability to communicate resident information to the Activities Director and other caregivers.
Ability to plan and carry out individual and group activities.
Ability to adapt activities to resident's cognitive and physical functional levels.
Ability to provide transportation to and from Activity Areas.
Ability to monitor and report safety, housekeeping, and maintenance issues.
Ability to maintain a clean and orderly activities space.
Ability to perform additional cleaning tasks as assigned.

Responsibilities

Gathers data from assigned residents to complete and update activities assessments.
Communicates resident information to the Activities Director.
Identifies ways to support resident's individual needs and preferences, customary routines, concerns and choices.
Carries out individual and group activities that meet the functional levels, needs and interests of each resident.
Provides activities to bed-bound residents and other residents confined to their rooms that reflect life-long interests.
Assists residents with activities as needed.
Provides transportation to and from Activity Areas.
Adapts activities to match the cognitive and physical functional levels of the residents.
Assists Director of Activities in developing activity calendars.
Posts calendars on resident halls and distributes these schedules to residents.
Monitors physical environment for safety or fire hazards and corrects or reports potential problems immediately.
Promptly reports housekeeping and maintenance problems.
Replaces all activities equipment and supplies in the proper storage areas.
Maintains activities space in a clean and orderly manner that appears "homelike" and is safe for the residents.
Performs additional cleaning tasks as assigned by Director of Activities.
