About The Position

This position is a full-time role located at the Green Bay Office. The Activities Coordinator oversees our Oshkosh and Green Bay Day Service programming, working as an active team member to support participants and staff. This role involves leading the implementation of Chance events and assisting with program objectives, focusing on HCBS compliance, planning, risk management, facilitation, and customer service. The job description is a general outline and subject to change based on business necessity. Overtime may be required as approved.

Requirements

  • High School Diploma, or equivalent, with requested years of direct care and administration experience will be considered.
  • Proficiency in Microsoft Suite Package, to include Excel, Outlook, Word, etc.
  • Excellent communication skills, written and oral.
  • Strong organizational skills.
  • Must be at least 21 years of age.
  • Must possess a valid WI driver’s license (where required), and ability to meet agency driving standards.
  • Must provide proof of auto insurance (where required).
  • Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Nice To Haves

  • A Bachelor Degree, in a Human Services or Similar Field, is preferred.
  • Experience in Human Services or related field may be considered.
  • Three years’ experience in direct care services and the administration of Adult Day Service programming caring for persons with disabilities, preferred.

Responsibilities

  • Ensure Clarity Care programs operate in compliance with laws, regulations, managed care organization contractual guidelines, and organizational policy.
  • Create and implement effective oversight systems for all programs supervised.
  • Evaluate quality metrics and outcomes achieved by Clarity Care on a monthly basis and undertake corrective actions.
  • Assess and evaluate staff development needs and ensure they are addressed through training and education.
  • Monitor physical plant and fire safety inspections and ensure all safety hazards are immediately addressed.
  • Supervise the financial management of Day Service programming, including fiscal forecasting, accounting principles, record keeping, and consumer accounts.
  • Monitor vacancy and occupancy levels to achieve goals.
  • Ensure full compliance with Wisconsin Department of Health Services Home and Community Based Services (HCBS) settings rules and perform self-audits.
  • Assist with transporting program participants to and from Day Service programming.
  • Plan, coordinate, and facilitate events to provide opportunities that meet customer needs and serve diverse interests and abilities.
  • Arrange all Chance event activities, including logistics, food, beverages, decorations, music, staff, volunteers, photography, and admission.
  • Provide input, feedback, and event ideas for long-term sustainability.
  • Research and consult with customers to determine their interests, event suitability, and attractiveness of offerings.
  • Prepare and maintain a registration database for all Chance events.
  • Assess customers’ fit within each event to ensure a cohesive group and mitigate potential conflict.
  • Provide a safe, memorable, and exceptional customer service experience that exceeds consumer expectations.
  • Respond promptly and competently in emergency response situations.
  • Assist, escort, and provide physical and other types of assistance needed by customers during events.
  • Promote and encourage the organization’s mission, vision, values, and philosophy.
