About The Position

This role involves assisting in the coordination and implementation of a therapeutic activity program for patients in a skilled nursing setting. The goal is to meet individual patient needs and interests through daily activities. Documentation of patient attendance, participation, and response to the program is a key part of the role.

Requirements

• High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required

Responsibilities

• Under the direction of the Activities Coordinator assists in coordinating and implementing a therapeutic activity program on a daily basis to meet the individual patient's needs and interests.
• Documents all duties performed with patients on a one-to-one and group basis, which indicates patient's attendance and participation in the program with reference to patients response to the program.
