About The Position

The purpose of Transportation Services is to increase or maintain the self-sufficiency of eligible individuals by providing access to human and community services through a coordinated transportation system. Transportation for elderly persons, especially those living in remote rural areas, provides a link to their communities to access needed services and for socialization. Transportation services make it possible to access the senior center, medical services, social services, shopping and recreation. Additionally, vehicles are used for the delivery of home meals. Priority is given to transporting individuals to and from the senior center and the delivery of home delivered meals. The driver is responsible for checking the board for cancellations and counting the meals going into the van. Participants that are new to the service are to be informed of the service, procedures for obtaining and scheduling a ride and be informed about the service contributions. Annually, this information should be made available to all participants.

Requirements

  • Minimum 18 years of age.
  • Valid Arizona operator's license or an ADOT approved alternate certification.
  • Licensed based on the type of vehicle operated.
  • Pre-service physical exam and in-service physical exam every two years thereafter.
  • Driving record checked with Arizona DOT, with no revocation or suspension of licenses during the past year.
  • Level One Fingerprint Card.
  • Provider agency Identification card.
  • Appropriate training in the use and operation of the vehicle and all vehicular equipment including wheelchair and ramps.
  • Training in First Aid and CPR.
  • Training in Fire Safety.
  • Training in Risk Management.
  • Training in Working with the Elderly and Disabled.
  • Training in Use & abuse of drugs and alcohol in/or affecting the workplace.
  • PASS Certified Program-Passenger and Service and Safety Driver Program every 3 years (Pinal-Gila requirement).
  • Keep all service records confidential and on file for five (5) years.
  • Maintain Participant Registration Card, with emergency information updated every six months or sooner if needed.
  • Obtain participant signature for receipt of each ride (or mark 'X' and have driver/escort sign participant's name and initial).

Responsibilities

  • Check the board for cancellations and count the meals going into the van.
  • Inform new participants about the service, procedures for obtaining and scheduling a ride, and service contributions.
  • Maintain vehicle trip logs including date, driver name, number of passengers, and mileage.
  • Maintain vehicle maintenance and repair reports.
  • Complete daily, pre-trip inspection logs.
  • Report accidents or incidents related to driver or vehicle safety.
  • Provide assistance to participants in boarding and disembarking the vehicle, allowing adequate time for safety.
  • Ensure adequate insurance coverage for vehicles.
