About The Position

The ACT Team Leader/SUD Therapist oversees and participates in the delivery of community-based mental health and substance use disorder services for adults with serious mental illness enrolled in the ACT program. This role provides clinical services while supervising ACT team staff to ensure quality care, regulatory compliance, and fidelity to the ACT model.

Requirements

  • Candidates must hold one of the following valid professional licenses in the State of Ohio and have a minimum of one year of experience providing substance use disorder (SUD) treatment:
  • Licensed Independent Social Worker (LISW)
  • Licensed Professional Clinical Counselor (LPCC)
  • Licensed Independent Marriage and Family Therapist (IMFT)
  • Licensed Psychologist

Responsibilities

  • Provide individual, group, family, case management, and counseling services within the scope of licensure and agency standards.
  • Complete intake and diagnostic assessments for new clients to determine eligibility for ACT services and complete updates as required.
  • Provide court representation, community probation support, and conditional release services when assigned.
  • Maintain knowledge of community resources and document appropriate referrals within client records.
  • Ensure services are delivered according to authorized frequency and duration and request additional authorization when necessary.
  • Develop and maintain Individualized Service Plans (ISP) with measurable goals in collaboration with the client and obtain required signatures at initiation and updates.
  • Ensure clinical documentation reflects client participation and meets agency and regulatory standards.
  • Complete progress notes within 24 hours of service delivery.
  • Notify consumers and referral sources prior to discharge when applicable and complete discharge summaries within 30 days of last contact.
  • Maintain accurate, organized, and compliant clinical records.
  • Collaborate with community agencies and providers to coordinate care and support client needs.
  • Maintain required professional licensure and continuing education.
  • Complete all required agency and regulatory trainings including client rights, safety, infection control, and other mandated trainings.
  • Adhere to ethical standards of practice and all organizational policies and procedures.
  • Participate in required clinical supervision in accordance with licensure requirements.
  • Serve as the ACT Team substance use counselor and ensure compliance with DACTS fidelity standards.
  • Facilitate weekly substance use treatment groups for ACT clients as needed.
  • Provide administrative and clinical supervision to Community Support Specialists
  • Review staff documentation, timesheets, and related personnel forms for accuracy and compliance.
  • Monitor productivity standards and service delivery expectations.
  • Conduct regular staff supervision sessions at least bi-weekly.
  • Evaluate staff performance, provide feedback, and monitor required trainings and certifications.
  • Collaborate with mangement to address performance concerns and implement corrective action plans when needed.
  • Review client charts to ensure compliance with documentation and regulatory standards.
  • Coordinate and review referrals to determine appropriate service placement.
  • Perform additional supervisory or clinical duties as assigned.
