The Access to Care Coordinator serves as a vital member of the maternal and infant health team, working in close collaboration with the Maternal Health Specialist to improve health outcomes for pregnant and postpartum individuals and their infants. While the Maternal Health Specialist focuses primarily on clinical assessment, education, and healthcare management, the Access to Care Coordinator focuses on reducing barriers to care, addressing social determinants of health, facilitating access to community resources, and supporting appointment adherence. This role provides intensive care coordination and patient navigation services for individuals identified as having elevated risk factors for adverse maternal or infant outcomes. The coordinator proactively engages patients, assesses patient needs, develops individualized support plans, and ensures patients have the resources necessary to access recommended care throughout pregnancy and the postpartum period.
