Requirements

High School Diploma or GED required
1–2 years of experience in a healthcare or behavioral health setting required
Strong organizational, communication, and customer service skills
Ability to work with diverse populations in a compassionate and professional manner
Comfortable using electronic health records and standard computer systems
Flexible, team-oriented, and able to manage multiple priorities

Nice To Haves

Associate’s Degree preferred
Knowledge of substance use, mental health, and care management preferred
Knowledge of community resources and how to access them

Responsibilities

Serve as the primary point of contact for individuals, ensuring consistent engagement and follow-through
Complete intake processes, assessments, and required documentation within the electronic health record
Collect and utilize Social Determinants of Health (SDOH) data to inform care planning
Coordinate referrals to internal programs and community-based services
Collaborate closely with clinical staff, care managers, and external providers
Maintain consistent communication with individuals throughout their connection to services
Support navigation into care management and recovery support programs
Ensure accurate documentation, tracking, and follow-up of all individuals served
