About The Position

The Access Coordinator often represents a client’s first interaction with the Coalition, this position plays a critical role in creating a welcoming, respectful, and solution-focused experience. The successful candidate must be able to engage individuals experiencing significant stress and crisis with empathy while maintaining professional boundaries, exercising sound judgment, managing multiple priorities, and consistently applying agency policies and program requirements. Using a housing-focused, trauma-informed, and problem-solving approach, the Access Coordinator conducts screenings and assessments, explores diversion and safe alternative housing options, and works with clients to identify natural supports and community resources that may prevent or quickly resolve an episode of homelessness. When diversion is not possible, the Access Coordinator facilitates access to appropriate shelter, housing programs, outreach services, and other supportive resources based on established eligibility criteria and availability.

Requirements

  • Associate’s degree in human services, social work, or related field; bachelor’s degree preferred.
  • Experience in diversion, case management, or social services, preferably in homelessness, housing, or crisis intervention programs.
  • Knowledge of community resources, local housing programs, and public assistance programs.
  • Strong communication and interpersonal skills, with ability to engage a diverse population.
  • Ability to work flexible hours, including evenings or weekends, as needed.
  • Proficiency in Microsoft Office and database management systems (e.g., HMIS).
  • Ability to work under pressure and manage multiple tasks simultaneously.
  • Strong problem-solving, critical thinking, and conflict resolution skills.
  • Effective written and verbal communication skills.
  • Proficiency in data entry, reporting, and basic computer applications.
  • Ability to establish rapport and maintain professional boundaries with clients.
  • Knowledge of trauma-informed and client-centered practices.

Responsibilities

  • Serve as the primary point of contact for individuals and families seeking shelter, diversion, or housing support services.
  • Conduct triage and diversion conversations, ensuring that shelter placement is considered only as a last option while exploring alternative solutions. Diversion strategies include relocation to family or friends, moving assistance, or other creative housing supports.
  • Conduct screening and intake assessments to determine client needs and eligibility for shelter.
  • Provide information, referrals, and crisis support, connecting clients to appropriate community resources and programs.
  • Document all client interactions, case notes, and service transactions accurately in HMIS within one business day along with uploading supporting documentation.
  • Collaborate with Housing Programs staff, Outreach teams, and community partners to ensure clients receive timely and coordinated support.
  • Maintain awareness of available housing options, diversion programs, and emergency resources to effectively guide clients.
  • Monitor and report on program metrics, outcomes, and compliance requirements as required by funders and program leadership.
  • Participate in team meetings, staff trainings, and professional development activities to maintain program knowledge and compliance.
  • Assist with data collection and reporting for grant and funder requirements.
  • Follow all organizational policies, procedures, and confidentiality protocols.
  • Perform other duties as assigned to support the effectiveness of the Diversion & Intake Department.
