About The Position

This position is responsible for actively navigating 911 callers to appropriate patient care across the health continuum. Our nurses telephonically assess patients and follow established, evidence-based protocols to match patient needs with available local and/or virtual resources. Additional duties include remote monitoring, medical screening, patient education, leadership opportunities, and mentoring as assigned. This role is part of the DOD SkillBridge program, offering active-duty U.S. Military members the opportunity to gain valuable civilian work experience in the medical response industry through training and internships during the last 180 days of their active-duty obligated service. The program runs for up to 180 days.

Requirements

  • Must be active-duty military to qualify for the SkillBridge program.
  • Minimum of 3 years nurse experience in direct urgent patient care.
  • Experience in Emergency or Acute Care settings.
  • Behavioral Health experience.
  • Care Coordination experience.
  • Maintain an active Nursing License (LVN, LPN) or RN Compact state Nurse License.
  • Maintain an active CPR card.
  • Proficiency in basic computer applications and functions.
  • Strong and effective verbal, written, and interpersonal communication skills.
  • Active listening with relevant follow-up questions.
  • Nursing skills as defined as generally accepted standards of practice.
  • Demonstrated patient teaching and educational facilitation skills.

Nice To Haves

  • Phone assessment and/or triage experience.
  • Compact License Nurse or RN License required.
  • Additional CA, DC, NY, MI, and/or OR licensure preferred.
  • Master’s or Bachelor’s degree in Healthcare Administration, Public Health or any other degree with a focus on community or disease education preferred.
  • Proficient Bilingual translations skills preferred.

Responsibilities

  • Actively navigate 911 callers to appropriate patient care across the health continuum.
  • Telephonically assess patients and navigate care based upon evidence-based protocols.
  • Perform remote monitoring.
  • Conduct medical screening.
  • Provide patient education.
  • Take on leadership opportunities.
  • Engage in mentoring as assigned.
  • Demonstrate the ability to function as a Registered Nurse or Licensed Nurse.
  • Demonstrate effective communication skills.
  • Work well with team members, patients, caregivers, referral sources, physicians and their staff, facility staff members and the community.
  • Effectively learn and use computer and required software programs.
  • Manage time, resources, and documentation requirements.
  • Provide excellent customer service.

  • Retention bonus ($2,000 for LVN/LPN, $4,000 for RN)
