About The Position

The Wound Care RN provides expert nursing care and education to patients with ostomies and wound care needs in a surgical hospital setting. Responsible for assessing, planning, and evaluating care for patients requiring ostomy management and other complex wound care procedures. This position places a strong emphasis on patient and staff education, ensuring optimal outcomes and improved quality of life for patients transitioning to self-care.

Requirements

Demonstrates eligibility for employment in the U.S.
Graduate of an accredited school of professional nursing (bachelor’s degree preferred)
Current licensure in the State of Wisconsin
Must be Wound Care Certified.
BLS certification is required upon hire
Ability to relate and work effectively with others
Two years of experience in wound care and ostomy education required.
Demonstrated skills in verbal and written English communications for safe and effective patient care and to meet documentation standards

Responsibilities

Assess, plan, implement, and evaluate nursing care for patients with ostomies, complex wounds, fistulas, drains, and skin integrity concerns.
Deliver one-on-one and group education to patients and families regarding ostomy management, wound care, appliance application, skin protection, and lifestyle adaptation.
Conduct pre- and post-operative ostomy teaching, including stoma site selection and ongoing stoma assessment
Collaborate closely with interdisciplinary surgical teams, providers, and case managers to implement and adjust wound/ostomy care plans
Document assessments, interventions, education, and outcomes in the electronic medical record (EMR) in accordance with hospital policy
Serve as a clinical resource and consultant for staff, providing education and training on ostomy and wound care best practices.
Participate in performance improvement initiatives related to wound care and patient outcomes.
Maintain inventory of wound and ostomy supplies; ensure proper utilization and cost-effectiveness.
Adhere to all safety protocols, infection prevention policies, and regulatory compliance standards.
Perform duties in accordance with hospital mission, vision, and values with a high degree of empathy and professionalism.
May be asked to work flexible shifts based on department needs.
