Wellness Coordinator Are you passionate about helping others live healthier lives? Join our team as a Wellness Coordinator , where your outreach and care coordination will directly support patients in accessing vital preventive services and improving their overall well-being. What You’ll Do Be the voice of wellness : Make outreach calls to patients, guiding them through eligible preventive care services like Medicare Annual Wellness Visits. Empower healthier choices : Educate patients on the importance of preventive care and help close care gaps that impact long-term health. Collaborate with care teams : Work side-by-side with physicians, nurses, and staff to create a seamless, integrated approach to patient care. Drive quality improvement : Support primary care practices in achieving better outcomes through documentation, process enhancements, and occasional presentations at staff meetings. Support care management : Assist with general office tasks, secretarial duties, and population health initiatives that make a real difference in our communities. What We’re Looking For A positive attitude and a team-first mindset. Strong communication skills —especially over the phone and via text. Comfort with technology , including Microsoft Office and electronic medical records. Ability to multi-task, prioritize , and stay organized in a fast-paced environment. A basic understanding of medical terminology is a plus. Qualifications Education : High school diploma or equivalent required; Associate Degree preferred. Experience : Prior clerical experience in a healthcare setting is preferred.
