As the Case Manager, Hospice, this employee is responsible for facilitating the patient through the continuum of care by coordination of services, programs and personnel. The role promotes the achievement of patient-centered goals with the use of appropriate resources and collaborates closely with system multidisciplinary teams to provide a coordinated experience for patients and families. This position participates in quality improvement and evaluation of processes. As the primary care nurse, this employee provides direct patient care including support and education of both the patient and family in the hospital. The nurse coordinates the case management among the hospice and hospital teams, utilizing additional resources as appropriate, in accordance with the mission, objectives, policies and procedures of the health system.
