About The Position

Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement. We are seeking an Inpatient Hospice Nurse to provide nursing care to patients who require GIP level of care for end-of-life symptom management and pain control in collaboration with the Stanford Hospital inpatient team. This RN will be stationed on the Stanford Hospital campus and requires a high degree of courtesy, tact, and knowledge of services. Bi-lingual in Spanish a plus!! Schedule: Saturday & Sunday; 8:30am-5:00pm Location: Stanford Hospital, Palo Alto Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • One year experience as a professional nurse in the past three years or; Bachelor’s degree in Nursing from an accredited School of Nursing
  • Current license as a registered professional nurse in the State of California

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred
  • Public Health Nursing certification highly desirable
  • HPNA certification preferred
  • Bi-lingual in Spanish a plus!!

Responsibilities

  • Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Stanford team and in accordance with CMS guidelines
  • Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
  • Perform evaluation and coordination of social work and spiritual support services needed by patient/family
  • Coordinate the implementation of the plan of care and ensure care coordination with the inpatient team
  • Maintain regular communication with the Stanford Hospital team and other team members involved in the Inpatient program
