About The Position

Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner or Physician’s Assistant to join the team at our Lake Region Walk-in Clinic. Join our fast-paced walk-in clinic team and provide high-quality, patient-centered care for individuals of all ages presenting with acute and episodic conditions, minor illnesses, and injuries following established guidelines, practices and protocols. The successful candidate will work collaboratively with a multidisciplinary team of to ensure optimal patient outcomes and satisfaction. Location: Honesdale, PA Employment Type: Part-time Shift: (2) 11-hour shifts per week, with rotating weekends and holidays

Requirements

  • Current, active, and unrestricted licensure as a Certified Registered Nurse Practitioner (CRNP) or Physician Assistant (PA) in the Commonwealth of Pennsylvania.
  • Graduation from an accredited Nurse Practitioner or Physician Assistant program.
  • National certification as an Advanced Practice Provider.
  • Current Basic Life Support (BLS) / Cardiopulmonary Resuscitation (CPR) certification;
  • Minimum 1-2 years of experience in walk in or family medicine required
  • Ability to work in a fast-paced environment, think critically, and make independent, decisive judgments.
  • Strong verbal and written communication skills, with a commitment to excellent customer service and compassionate care.
  • Skill in applying and modifying the principles, methods, and techniques as a professional health care provider; in taking medical history to assess medical condition and interpret findings
  • Ability to maintain quality control standards and ability to react calmly and effectively in emergency situations
  • Ability to interpret, adapt and apply guidelines and procedures and to communicate clearly

Responsibilities

  • Patient Care: Conduct complete, detailed, and accurate medical histories and physical examinations for walk-in patients
  • Diagnosis & Treatment: Formulate appropriate diagnoses and develop and implement individualized treatment plans within the scope of practice and established protocols.
  • Diagnostics: Order and interpret diagnostic tests, as needed.
  • Procedures: Perform minor in-office procedures, as needed
  • Documentation: Maintain accurate, timely, and confidential patient records using our integrated Electronic Health Record (EHR) system (eClinicalWorks)
  • Education & Counseling: Provide comprehensive health education and guidance to patients and families regarding preventative health, treatment plans, and side effects.
  • Collaboration: Consult with supervising physicians on patient care and administrative issues, collaborating regularly with nursing and medical staff to ensure quality care.
  • Quality Improvement: Actively participate in patient safety and quality improvement processes.
  • Performs other duties as assigned to support efficient operation of the department.
