About The Position

Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner or Physician’s Assistant to join the team at our Lake Region Walk-in Clinic. Join our fast-paced walk-in clinic team and provide high-quality, patient-centered care for individuals of all ages presenting with acute and episodic conditions, minor illnesses, and injuries following established guidelines, practices and protocols. The successful candidate will work collaboratively with a multidisciplinary team of to ensure optimal patient outcomes and satisfaction. Requirements

Current, active, and unrestricted licensure as a Certified Registered Nurse Practitioner (CRNP) or Physician Assistant (PA) in the Commonwealth of Pennsylvania.
Graduation from an accredited Nurse Practitioner or Physician Assistant program.
National certification as an Advanced Practice Provider.
Current Basic Life Support (BLS) / Cardiopulmonary Resuscitation (CPR) certification;
Minimum 1-2 years of experience in walk in or family medicine required
Ability to work in a fast-paced environment, think critically, and make independent, decisive judgments.
Strong verbal and written communication skills, with a commitment to excellent customer service and compassionate care.
Skill in applying and modifying the principles, methods, and techniques as a professional health care provider; in taking medical history to assess medical condition and interpret findings
Ability to maintain quality control standards and ability to react calmly and effectively in emergency situations
Ability to interpret, adapt and apply guidelines and procedures and to communicate clearly

Responsibilities

Patient Care: Conduct complete, detailed, and accurate medical histories and physical examinations for walk-in patients
Diagnosis & Treatment: Formulate appropriate diagnoses and develop and implement individualized treatment plans within the scope of practice and established protocols.
Diagnostics: Order and interpret diagnostic tests, as needed.
Procedures: Perform minor in-office procedures, as needed
Documentation: Maintain accurate, timely, and confidential patient records using our integrated Electronic Health Record (EHR) system (eClinicalWorks)
Education & Counseling: Provide comprehensive health education and guidance to patients and families regarding preventative health, treatment plans, and side effects.
Collaboration: Consult with supervising physicians on patient care and administrative issues, collaborating regularly with nursing and medical staff to ensure quality care.
Quality Improvement: Actively participate in patient safety and quality improvement processes.
Performs other duties as assigned to support efficient operation of the department.
