Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner or Physician’s Assistant to join the team at our Hamlin Walk-in Clinic. Join our fast-paced walk-in clinic team and provide high-quality, patient-centered care for individuals of all ages presenting with acute and episodic conditions, minor illnesses, and injuries following established guidelines, practices and protocols. The successful candidate will work collaboratively with a multidisciplinary team of to ensure optimal patient outcomes and satisfaction. Location: Lake Ariel, PA Employment Type: Full-Time Shift: 3.5 days per week (2- 11 hour shifts, and 2- 7-hour shifts) with rotating weekends and holidays What you will do as a WMCHC Nurse Practitioner/Physician’s Assistant: Patient Care: Conduct complete, detailed, and accurate medical histories and physical examinations for walk-in patients Diagnosis & Treatment: Formulate appropriate diagnoses and develop and implement individualized treatment plans within the scope of practice and established protocols. Diagnostics: Order and interpret diagnostic tests, as needed. Procedures: Perform minor in-office procedures, as needed Documentation: Maintain accurate, timely, and confidential patient records using our integrated Electronic Health Record (EHR) system (eClinicalWorks) Education & Counseling: Provide comprehensive health education and guidance to patients and families regarding preventative health, treatment plans, and side effects. Collaboration: Consult with supervising physicians on patient care and administrative issues, collaborating regularly with nursing and medical staff to ensure quality care. Quality Improvement: Actively participate in patient safety and quality improvement processes. Performs other duties as assigned to support efficient operation of the department.
