About The Position

Carefree Living in Burnsville is seeking a Waiver Support Coordinator to join our growing and established team. Salary: $55,000 per year Location: Burnsville, MN Schedule: Full-time position. This role will begin working on-site , with the potential to move to a hybrid schedule based on performance and operational needs. Occasional travel to other Carefree Living locations may be required.

Requirements

  • Working knowledge of Elderly Waiver (EW), 67/90 waivers, and other applicable waiver programs required
  • Strong organizational and time management skills with the ability to manage multiple deadlines
  • Excellent communication skills and ability to work collaboratively with multiple teams and external partners
  • Detail-oriented with strong follow-through skills
  • Proficiency with computer systems and Microsoft Office required
  • Ability to travel occasionally between locations as needed
  • Selected candidate must be able to pass a DHS background study

Nice To Haves

  • Experience working with Medicaid-funded services in assisted living, home care, or similar settings preferred

Responsibilities

  • Monitor waiver expiration dates for residents across multiple Carefree Living locations
  • Track upcoming renewals and notify site leadership of approaching deadlines
  • Coordinate required assessments and ensure appointments are scheduled in a timely manner
  • Communicate with county case workers to confirm requirements and maintain compliance
  • Follow up with site teams to ensure all required documentation is completed and submitted
  • Assist with gathering necessary records and paperwork related to waiver renewals
  • Maintain accurate tracking systems to ensure all waiver steps are completed prior to expiration
  • Work collaboratively with clinical teams and administrative staff to ensure continuity of services and payment
  • Provide updates to leadership regarding waiver status and outstanding items
  • Support process improvement efforts related to waiver tracking and coordination
  • Other duties as assigned
