Requirements

  • A high school diploma or equivalent certificate is required; an associate's or bachelor's degree in an applicable field is preferred.
  • Experience in rehabilitation, employment services, case management, social services, or related fields is preferred.
  • Successful completion of background checks.
  • Must have current driver's license with an acceptable driving record and auto insurance.

Responsibilities

  • Treat all clients, referral sources, stakeholders, and team members with dignity and respect.
  • Develop and maintain a working knowledge of disabilities and potential assistive technology/accommodations.
  • Compile and review vocational, medical, psychological, and educational information to assist clients with skill development.
  • Contribute to and/or develop individualized service plans with clients.
  • Conduct activities identified within clients’ individualized service plans.
  • Coordinate paid, work-based learning experiences, including orientation and paperwork.
  • Transport clients to meet goals identified in their service plans.
  • Monitor and report on progress toward individualized service plan goals.
  • Ensure authorizations for services are current.
  • Instruct and facilitate clients’ skill development in vocational, social, and independent living contexts.
  • Develop strategies and curriculum for skill development based on individualized service plans.
  • Establish community-based sites for skill development and career planning.
  • Provide onsite job supports to assist clients with their job duties.
  • Educate employers regarding the benefits of hiring persons with disabilities.
  • Serve as a liaison between employers, clients, and co-workers.
